General Documentation Guidelines

Before starting your service request with us, we recommend keeping the following documents ready. On case to case basis, some additional documents may also be needed.

General Documents (Required for Most Services)

Service-Specific Documents

OCI Application

  • Birth Certificate (for minors)
  • Parent's Indian Origin Proof
  • Marriage Certificate (for spouse-based OCI)
  • Proof of Renunciation (if applicable)
  • Voter ID/Ration Card surrender Affidavit.

Passport Renewal

  • Copy of expiring passport
  • Name Change Affidavit (if applicable)
  • New U.S. address proof
  • Police Report – Not more than 6 months old
  • Annexure C/D/E/F(as applicable)

Visa Services

  • Travel Itinerary
  • Invitation Letter (for Entry/Business)
  • Employment Letter (if required)

Certificate Attestations (Birth/Marriage/Death)

  • Original Certificate + Copy
  • Relevant Passport Copies (self or parents/spouse/deceased)

Common Affidavits(MUST be Notarized)

  • Name Change Affidavit
  • Change of Appearance and Signature Affidavit for Passport
  • Sworn Affidavit for OCI/Passport
  • Consent Letter for Minor's Passport/OCI
  • Lost/Damaged Passport Affidavit
  • Relationship Affidavit (if needed)
  • Single Status Affidavit (for marriage)

  • All non-English documents must be translated into English.
  • All pages to be self-attested, and all copies to be notarized.
  • Originals may be required during verification.
  • Indian Embassy/Consulates may request more documents case-by-case.
